Billing Policies for UWRF Speech and Hearing Clinics
UWRF Speech and Hearing Clinics is a fee-for-service clinic. All services are provided by graduate clinician(s) with direct supervision of a certified speech-language pathologist and/or audiologists. Our goal is to provide professional services, provide a wide variety of training and education to our students, and to provide clients services that are available to all clients at an affordable cost. All clients will be charged the same fee for services. We do provide discounts or free services for those who qualify under our Educational Purposes Only Criteria or the National Federal Poverty Level Sliding Fee Scale set by our clinics.
Billing Third Party Insurance
We are in-network providers for the following insurance carriers:
- Anthem/BlueCross Blue Shield
- Cigna
- HealthPartners
- Preferred One
- Outreach Health and iLife (ISIS payment plans)
If we are a provider and submit claims to your insurance, we will also need to have:
- A current copy of your insurance card (front and back)
- A signed release to bill insurance on file (this must be done yearly)
Additional notes for insurance claims:
- Some insurances require prior authorization for therapy services so we will need to obtain this before therapy services can be rendered. Our billing specialist will work with your primary physician and insurance to obtain the prior authorization and communicate with you whether your insurance approves services and discuss options if it is a non-covered service
- Agreement to bill your insurance and even obtaining prior authorization does not ensure payment for services rendered.
- As a provider of the previously listed insurances and due to our contracts with them, we will honor any discounted contractual amounts per your insurance. In turn, you as the client will be responsible and billed for any portion that your insurance deems billable to the patient due to deductibles, co-pays, co-insurance amounts or non-covered services
Educational Purposes Only
- Occasionally we have clients with a diagnosis that is beneficial for our students for the purpose of variety of training and education
- To be seen for educational purposes only, our billing specialist will provide a form stating that you are being seen for educational purposes only per the Clinic’s needs and clients will need to requalify for this yearly
Private Pay
All clients whose policy is not listed above or whose insurance policy does not cover our services will be considered Private Pay. All Private Pay clients and insurance clients are billed for services based on the clinic’s established fee schedule.
Sliding Fee Schedule:
- Please note that all Private Pay clients do have the option to qualify for reduced or free services based on a sliding fee scale
- This scale is set by federal guidelines for Federal Poverty Level
- To qualify you will be required to do additional paperwork, provide verification, and meet with our Billing Specialist prior to services
Payment Plans:
- We will work with clients to provide payment plans
- We do offer a 50% discount if full payment for the semester is paid on the last appointment of the semester. *This discount is possible as payment in full eliminates invoicing costs, mailing costs, claim submission costs, and collection follow-up, therefore lowering our overhead.
Speech-language and hearing services may be paid for by check, cash (no change is available so it must be the exact amount), or credit card.
We have a billing specialist, Lara. Please feel free to reach out to her if you have any questions or concerns at (715) 425-3233 or you can email her at uwrfclinic@uwrf.edu and put billing in the subject line.
